INTAKE SERVICES AT CHRYSALIS CENTER, INC.

Chrysalis Center Inc. offers a centralized prescreening and admission process. Referrals are typically initiated by Health Professionals or Social Service agencies, but individuals can stop in for assistance themselves at our Intake office located at

278 Farmington Avenue, Hartford, CT.

Referrals to our Residential Community Case Management Services and our Community Employment Service must be submitted to Capital Region Mental Health Center.
In addition to the particular service application, we may ask for other records, including
- psycho-social assessment - police records
- substance abuse assessment - other medical documentation

    AFTERCARE

Aftercare services are offered to individuals being discharged from Chrysalis Center, Inc
services. This unique and critically important service provides four follow-up contacts to the discharged individual that take place at one month, three months, six months and one year after discharge. The purpose of Aftercare is to provide support to individuals and their families in order to promote a successful transition out of our services, as well as to link or refer to other services in the community as needed. Aftercare services can also address recidivism by providing timely interventions and support prior to a crisis or re-hospitalization. Aftercare services are provided by the individual’s Case Manager.
