Community Case Management Services
Community Case Management Services are designed to assist individuals in meeting critical health, recovery, rehabilitative and other individual-identified life goals including: obtaining/maintaining safe and affordable housing, managing life tasks essential to living in the community, managing mental and physical health care needs, obtaining employment and other vocational goals, living in an integrated manner in the community of choice in the least restrictive setting, creating and sustaining natural, culturally sensitive supports in the community.
Residential Community Case Management
Residential Community Case Management provides flexible wrap–around supports tailored to meet the needs of adults 18 years of age, or older, who have low income and a primary diagnosis of severe or prolonged mental illness. Our goal is to assist clients in acquiring and maintaining safe, affordable housing while living successfully in the community. Services are available to individuals in Hartford and West Hartford and include medication and health education, life skills training and individualized wrap-around support services.
Community Resource Services
Community Resource Services uses a self-help, peer support model in providing community case management to adults 18 years of age, or older, who have low income and a primary diagnosis of mental illness. The service assists clients in obtaining and maintaining decent, affordable housing, while providing individualized support services, including self advocacy and peer support. The goal of the service is to assist clients in living as independently as possible in the community. CRS focuses and emphasizes hope, self-advocacy and self direction. Services are available to individuals living in West Hartford, Simsbury, Farmington, Avon, and Canton.
Community Integration Services
Community Integration Services, funded through the Department of Mental Health and Addiction Services, is a collaborative effort between Chrysalis Center, Inc and the Forensics Unit of Capitol Region Mental Health to provide community support services to individuals who have been diagnosed with a mental illness or chronic substance abuse and who have outstanding criminal justice needs. Services include a needs assessment and recovery planning assistance, housing and employment supports, linkage with appropriate community resources, peer support and supportive counseling to promote stability during the recovery process. Services are available to individuals receiving services through any of the programs associated with Forensics Services at Capitol Region Mental Health in Hartford.
All Case Management Services include:
Housing:
• Assistance in finding and obtaining safe, decent and affordable housing
• Linkage to rental assistance for qualified individuals
• Assistance with maintaining housing, including mediation with landlords
Support Services:
• Assistance in securing entitlements (i.e. Social Security, SAGA, Basic Needs)
• Assistance in money management
• Assistance in developing & maintaining activities of daily living such as cooking, cleaning, etc.
• Individualized Recovery Plans
• Referral to support and recreational services
• Linkage to employment and education supports
• Transportation services & training in the use of public transportation
Medical:
• Assistance in accessing and coordinating medical, substance abuse, and mental health care services
• Medication and health education services

Community Transitional Services
Community Transitional Services provide flexible wraparound case management supports tailored to meet the needs of adults 18 years of age, or older, who are being released from prison, on Parole or Transitional Supervision. Our goal is to increase independence and self reliance so that clients can become contributing community members by providing transitional housing, board and care, for up to four months while assisting the client to obtain employment; acquire safe, affordable housing; and meet the terms of their parole or transitional supervision release. Services are available up to 18 individuals in the transitional apartments located in Hartford.
