Community Supportive Housing
Supportive Housing Services provides permanent, independent housing that is safe and affordable to individuals who are homeless , especially those who have special needs including mental illness, HIV/AIDS and/or substance abuse histories. The goal of supportive housing is to enable formerly homeless individuals to live stable, independent lives by providing permanent housing with support services.

Single Site Programs:

Hudson View Commons:   This permanent supportive single site is located on Hudson Street in Hartford.  The mixed tenancy of this 28-unit building is comprised of a mix of low income or individuals that were formerly homeless with a primary diagnosis of any of the following:  Severe mental illness, substance abuse and/or HIV/AIDS.  Services are available to all in the building and include case management, employment linkages and supportive counseling.  Contact:  Pre-Admissions Assistant at (860) 263-4400

Soromundi Commons:  Translated from Swahili, Soromundi means “Sisters of the World.”  This project is a collaboration between Chrysalis Center, (case management services), Konover Properties, (property management services), and the YWCA of Hartford (building owner). This unique project houses three programs at this one site: 

  • A Women’s 15-bed Homeless Shelter: A three month program for women who are homeless, 18 years and older;
  • 2.  A two year Transitional Living Program (TLP) that is comprised of 13 units for single adult men and women;
  • 3.  Permanent supportive housing .which services 35 single adults who are low income and/or formerly homeless with a mental illness or substance abuse disability in one bedroom apartments.

Scattered Site Programs:

Veterans Support Services:  This program serves 25 Homeless Veterans, providing housing and targeted case management subsidies to both individuals and head-of-household Veteran families, in Hartford and the Hartford Region.  Contact:  Pre-Admissions Assistant at (860) 263-4400

F.U.S.E.: “Frequent User Service Enhancement” provides enhanced case management and housing subsidies, with a particular focus on employment, to ten single adults in Hartford.  Participants have been identified by a unique data-matching process as having significant experiences of homelessness and multiple jail admissions.  Contact: Pre-Admissions Assistant at (860) 263-4400

C.H.H.S.:  Community Housing and Health Services provides flexible supports to individuals and families diagnosed with HIV/AIDS.  Services focus on reducing homelessness, helping clients obtain and keep safe, affordable housing, accessing health care services, providing health education and assisting clients and their families in coping with the effects of the illness.  Services are available in Hartford, East Hartford, Wethersfield, Bloomfield, and West Hartford.  Contact: Pre-Admissions Assistant at (860) 263-4400

Family Matters:  This program assists single parents and their dependent children by providing support services and housing subsidies.  All families come from homelessness and may have mental illness, HIV/AIDs and/or substance abuse.  Services are available in Hartford, West Hartford, Bloomfield and Wethersfield.  Contact:  Pre-Admissions Assistant at (860) 263-4400

Project H.E.A.R.R.T.:  “Housing Employment Alternatives and Risk Reduction Team”is a program that provides linkages to housing subsidies, case management and supportive counseling.  Services are provided to single adults who were previously homeless, struggling with substance abuse and/or mental illness, HIV/AIDS.  Pre-Admissions Assistant at (860) 263-4400

Next Steps:  This program assists individuals in Hartford and the Greater Hartford Region with housing subsidies and case management for those who meet the criteria of having severe or prolonged psychiatric disabilities and/or substance abuse, who were chronically homeless or at risk of becoming homeless.  Contact:  Pre-Admissions Assistant at (860) 263-4400

HPRP:   “Homeless Prevention and Rapid Re-Housing Region 4” provides temporary financial assistance and housing relocation & stabilization services for those that would be homeless but for this assistance.  Support is available for those eligible in the Capitol Region (excluding Hartford and New Britian).  Contact:  Hotline Number:  860-263-4489
