Community Supportive Housing

 Supportive Housing Services provides permanent, independent housing that is safe and affordable to
individuals who are homeless , especially those who have special needs including mental illness, HIV/AIDS
and/or substance abuse histories. The goal of supportive housing is to enable formerly homeless
individuals to live stable, independent lives by providing permanent housing with support services.

Hudson View Commons Supportive Housing

Hudson View Commons (HVC) is a national demonstration project for the Corporation for Supportive Housing. This 28
unit building houses tenants who were formerly homeless and who have a primary diagnosis of any of the following:
severe mental illness, substance abuse and/or HIV/AIDS. The other non-disabled tenants are individuals with low income.
The goal of the services is to assist the tenants in maintaining permanent housing. The services, available to
all 28 tenants, includes case management, supportive counseling and employment supports

Community Health & Housing Services

Community Health & Housing Services provide flexible supports to individuals and families diagnosed with
HIV/AIDS. Services focus on reducing homelessness, helping clients obtain and keep safe, affordable housing, accessing
health care services, providing health education and assisting clients and their families in coping with the effects of
the illness. A primary goal for individuals is to live as independently as possible in their community. Services are
available to individuals and families in Hartford, East Hartford, Wethersfield, Bloomfield and West Hartford.

Family Matters Supportive Housing

Family Matters provides support services and housing subsidies to single mothers with dependent children under the
age of 18 years old. All families have come from homelessness and all may have mental illness, HIV/AIDS and/or
substance abuse issues. Services are available in the towns of Bloomfield, West Hartford and Wethersfield.

Project H.E.A.R.R.T. Supportive Housing

(Housing, Employment, Alternatives and Risk Reduction Team)

Chrysalis Center is the lead organization in this collaborative project comprised of several Greater Hartford based
organizations. Chrysalis Center and its partners provide linkage to housing subsidies, case management, and supportive
counseling to individuals who are 18 years of age, or older, who were previously homeless, struggling with substance
abuse, mental illness or HIV/AIDS.

Soromundi Commons

Soromundi Commons is a supportive housing project developed in collaboration with the YWCA of the Hartford Region.
Soromundi, meaning

“sisters of the world”, provides 71 units of housing including emergency shelter, transitional and
permanent supportive housing units, all under one roof.
